Competitive Exam Details

The Union Public Service Commission Combined Medical Services (UPSC CMS) examination is the most prestigious, highly coveted, and authoritative recruitment drive for medical graduates in India. For MBBS professionals seeking a stable, powerful, and socially impactful career outside of the intensely commercialized private healthcare sector, the CMS exam is the ultimate gateway. Successful candidates are recruited directly into Central Government organizations as Medical Officers. The posts offered include Assistant Divisional Medical Officer in the Railways, General Duty Medical Officer (GDMO) in the Central Health Service, GDMO in the New Delhi Municipal Council (NDMC), and Medical Officers in the Ordnance Factory Board. These are elite Group 'A' and Group 'B' gazetted posts offering a starting Level-10 salary (equivalent to an IAS or IPS officer), Non-Practicing Allowance (NPA), incredible job security, and unparalleled medical benefits.

The selection process is highly streamlined to assess the clinical, surgical, and preventive knowledge of the candidates. It comprises a Computer Based Test (CBT) and a Personality Test (Interview). The CBT is divided into two distinct objective-type papers, each containing 120 questions (250 marks) to be completed in 120 minutes. Paper-I primarily evaluates the candidate's proficiency in General Medicine and Paediatrics. Paper-II encompasses Surgery, Gynaecology & Obstetrics, and Preventive & Social Medicine (PSM). Because the exam tests the entire 5.5-year MBBS curriculum in a single day, the breadth of knowledge required is monumental. A strict 1/3rd negative marking penalty is applied for every incorrect answer.

Candidates who clear the rigorous CBT cut-offs are summoned to the UPSC headquarters in New Delhi for the Personality Test, which carries 100 marks. The interview panel evaluates the candidate's general knowledge, leadership qualities, medical ethics, and ability to handle high-pressure clinical or administrative scenarios. Final merit is formulated by combining the marks of the CBT (500 marks) and the Interview (100 marks). For doctors looking to serve the nation on a macro scale—whether managing railway hospitals or shaping public health policies—UPSC CMS is the absolute pinnacle.

Q1: Can final-year MBBS students apply for the UPSC CMS examination?
A1: Yes. Candidates who have appeared or will be appearing in the final MBBS examination can apply. However, they must show proof of having passed the written and practical parts of the final MBBS exam before they can be appointed. They also need to complete their compulsory rotating internship.
Q2: What is the maximum age limit to apply for the UPSC CMS exam?
A2: A candidate must not have attained the age of 32 years as of 1st August of the exam year. However, for Medical Officers in the Central Health Service, the upper age limit is relaxed to 35 years. Standard category-based relaxations (SC/ST/OBC) also apply.
Q3: Is there a descriptive written paper in the CMS exam?
A3: No. Unlike the UPSC Civil Services exam, the written component of the Combined Medical Services exam is entirely objective (Computer Based Test with Multiple Choice Questions).
Q4: Is the Non-Practicing Allowance (NPA) provided to all CMS officers?
A4: Yes. Since government medical officers are prohibited from running private clinics or engaging in private medical practice, they are compensated with a lucrative Non-Practicing Allowance (NPA), which is added to their basic pay and is also calculated for Dearness Allowance (DA) and pension.
